At UNLV Health our Health Information Department is dedicated to maintaining a high level of privacy and confidentiality with all patient records. We keep all health information private and secure in accordance with federal and state regulations.

If you are a patient and would like to request your medical records, please complete the appropriate release form. For your convenience, you may fax the completed request to us by mail or fax (see below). Please include a copy of your state ID, driver’s license and/or passport, as we need to verify your identity prior to releasing a copy of your medical record.
